Safety Reporting

Report a safety concern with clear operational context

Submit hazards, close calls, procedural concerns, inspection findings, maintenance issues, and safety observations for PlaneCare review.

Emergencies and accidents

This PlaneCare form is not an emergency channel and is not a substitute for required FAA, NTSB, airport, operator, or company reporting. For an active emergency, contact the appropriate emergency response authority immediately. Aircraft accidents should be reported through the required official channels.

Comparable aviation reporting model

PlaneCare’s form structure is informed by public aviation safety reporting patterns, including NASA ASRS and FAA safety reporting guidance.

  • NASA ASRS separates reporter identity from the event report and requests event type, occurrence date/time, conditions, aircraft/operator context, location, conflicts, and a narrative.
  • FAA’s safety page routes aviation safety issues to ASRS, FAA Hotline, bird strike, laser incident, NTSB accident reporting, and malfunction/defect reporting resources.
